Investigation Reveals Technical And Cultural Failures In Crewed Mission
NASA has formally labeled Boeing’s first crewed Starliner mission a Type A mishap, the agency’s most serious safety classification, after a test flight meant to last about two weeks stretched into a months-long saga. New NASA Administrator Jared Isaacman faulted both Boeing and NASA for the issues, citing technical problems and cultural breakdowns. The mission, which launched on June 5, 2024, was intended to last eight to 14 days but extended to 93 days due to propulsion system anomalies. The investigation found a mix of hardware failures, gaps in qualification work, leadership missteps, and cultural breakdowns. According to AeroTime, NASA and Boeing have continued working to identify and address the issues, with root-cause analysis still underway.
