The National Transportation Safety Board has concluded that a midair collision between a PSA Airlines CRJ700 and a US Army Black Hawk helicopter near Washington’s Reagan National Airport was caused by a cascade of systemic failures. The Safety Board found that flawed airspace design, inadequate oversight, and breakdowns in air traffic control and helicopter operations all contributed to the crash, which killed 67 people.
